DESCRIPTION: This position involves working with individuals with severe and persistent mental illness. Roles and responsibilities include but not limited to completing community assessment to assess goals, ensure consumer receive timely continuity of care, assisting with individual service coordination and linkages, consumer advocacy, training in daily living skills, direct assistance with structuring and performing ADL, consumer psychoeducation and support services, crisis intervention services, medication education and health care monitoring and oversight services. ADDITIONAL REQUIREMENTS: Required - Bachelor's Degree in helping profession and 1 year of experience working with adults in the mental health field. A current driver's license issued in the driver's state of residence is required without restrictions and with all necessary endorsements appropriate to the class of vehicle being operated. CPR Certification, computer literate, Crisis Intervention Training, organizational ability, clinical skills, excellent communication and interpersonal skills are required.
