The Health Behavior Coordinator (HBC) works with Primary Care providers and other health care providers to assist in improved patient-centered care communication and utilization of Motivational Interviewing. HBC serves as a valued member on the Healthy Living Team, which engages in administrative and clinical programming of promotion of health and prevention of disease. This position includes clinical care time , which is provided in our Primary Care Mental Health Integration (PCMHI) clinic.
